Care Co-ordinator

We are seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a crucial role in ensuring that our clients receive the highest level of care and support.

 

You will be responsible for coordinating and managing the rostering of our clients, working closely with healthcare professionals, caregivers, and clients themselves.

​

Responsibilities:

- On-Call

- Managing weekly rota's to ensure care calls are organised and well planned by using rostering software.

- Answering and actioning phone calls

- Liaising with the recruitment officer to fulfil rostering needs.

- Monitoring and actioning emails.

- Handing over information with relevant colleagues and healthcare professionals

- Act as a liaison between clients, caregivers, and healthcare providers to ensure effective communication and collaboration.

- Provide guidance and support to caregivers, offering training and resources to ensure they provide the best care possible.

- Monitoring business performance and reporting to management.

- Confirming visits for finance.

- Respond promptly to client and caregiver inquiries and concerns.

- Ensure compliance with all relevant healthcare regulations and standards.

Qualifications:

- Minimum Level 3 Health and Social Care

- Previous experience in care coordination or a similar role in healthcare.

- Strong communication and interpersonal skills.

- Excellent organiSational and problem-solving abilities.

- Compassionate and empathetic attitude toward clients and their families.

- Knowledge of healthcare regulations and standards.

- Proficiency in using healthcare management software and tools.

The main duties of a Domiciliary Health Care Assistant will include:
- Providing personal care, including but not limited to - washing, dressing, nutrition and hydration
- Providing social, emotional and physical support to clients
- Domestic duties such as dusting, vacuuming and changing bed linens
- Meal preparation
- Support with activities
- Support with medication needs
- Companionship

​

The ideal candidate will:
- Have an NQV Level 2 or above in health and social care or be willing to work towards one. - Have previous relevant training to the job role or be willing to attend training.
- Have previous domiciliary healthcare experience (not essential)
- Have a DBS check dated within 3 months or be prepared to register for one
- Be flexible, reliable and committed
